Patient Information
Please bring all medicines you are taking to every appointment. For your first appointment, we will also need you to bring your insurance cards, a referral from your primary care physician (if required by your insurance company), and any previous medical records.

Insurance
We accept assignment from Medicare and Arkansas Medicaid and are participating providers with most insurance companies. We file all insurance unless otherwise requested. We file insurance as a courtesy. Balances not paid by insurance after 45 days will be billed to the patient. Deductibles and co-insurance amounts are collected at the time of service. NWA Heart & Vascular Center is in compliance with Title VI of the Civil Rights Act.
